The following fee schedule represents the discounted one-time
fee that plan attorneys will charge our members for the
corresponding listed legal service. Compare the difference
between member rates and the national average charged by
law firms for the same service.
The savings is tremendous!
In the event a member requires extended legal representation,
our plan member attorneys have contracted with us to never
charge more than $75.00 per hour for in or out-of-court
representation. In most cases, the attorneys will be able
to estimate the amount of hours any given case may consume
prior to beginning work on the case.
After the free letters, calls and consultations take place,
members and their attorneys will have to decide what course
of action to take next. In most cases, members will have
no choice and litigation may ensue. This is the point in
time that Legal Club plan attorneys charge the $75.00 hourly
rate for legal care. This represents up to a 66% savings
from our competitors insurance plans. Our competitors in
the legal expense insurance business offer their members
a 25% discount off hourly rates. In California, Florida,
Texas, New York and other large states, this will leave
their members paying well over $200.00 per hour for extended
legal care. The savings with Legal Club will be thousands
of dollars. In fact, competitors required retainers will
be thousands more than Legal Club because retainers are
figured using the attorney’s hourly rate and estimated
time needed to service the member.
Contingency fee will be a 10% reduction of the state maximum
rate or the attorney’s usual rate, whichever is
lower.
All areas are included in our plan. Legal Club does not
exclude any type of law area. Many legal expense insurance
products have waiting periods, pre-existing condition
exclusions and limitations on usage. This is due to the
fact that they have financial exposure to usage, just
the same way an HMO has. As such, many types of law areas
are excluded in their plans. Legal Club has no such exposure
and as such, takes the opposite approach and encourages
usage. In fact, 80% of all new members use the legal
plan within the first 90 days they join.

ABOUT OUR ATTORNEYS
Our
international network of over 20,000 attorneys is professionally
managed. Attorneys in our network average 19.4 years of experience.
All plan-member attorneys must have a "BV" or better
rating if listed in Martindale-Hubbell, provide us with a
certificate of liability insurance, whether their state requires
it or not, be in good standing with their respective state
bar, have a law office in their respective state and carry
a "Courtesy Rating" of "B" or better.
In addition, Legal Club files a quarterly background disciplinary
report with each state's attorney disciplinary department.
Courtesy ratings as well as
background checks are done every three months. In addition, attorney firms must complete quarterly
surveys reaffirming the accuracy of our database and knowledge
of the plan. Besides Courtesy Rating surveys and quarterly
background checks, we answer to our members. Problems that
arise with a particular attorney are documented and can ultimately
lead to expulsion from our network of attorneys.
Our attorneys abide by a discounted
fee schedule in order to receive member referrals. For example, planmember attorneys
provide Individual / Family Legal Plan members with 7 Free
Services, charge a one-time fee for 8 Commonly Used Legal
Services, give members a 10% discount on all contingency
based cases and contract to charge no more than $75.00 per
hour for any and all extended legal care.
Legal Club of America is not
an insurance company and we have no financial relationship
with our attorneys. This means
that all of our legal plans have no exclusions, limitations,
claims, or red tape. We provide the only legal plans in the

Guaranteed Fixed Hourly Rates:
In the event

At a cost of $124.00 per year, it's a benefit your family cannot afford to

USING THE LEGAL PLAN
All your legal needs, new or old, can
now be attended to. Due to the volume of clients that Legal
Club of America® refers to its participating attorneys,
these attorneys have agreed to abide by the discounted
fee schedule and provide the free services described in
this guide book to Legal Club members. If you encounter
any problems accessing these services, please contact our
member service center for assistance or reassignment if
necessary.
YOUR PLAN ATTORNEY
When a new legal problem arises, call your participating attorney during
normal business hours. His or her name and number are on your Legal Club
welcome letter. All participating attorneys are pre-qualified, licensed
and registered to practice law in your state. They will be there to assist
you with your legal problem.
Only participating attorneys are obligated to provide our plan members
with our low discounted fees. Personal and family legal situations are
eligible for these fees. Many of the attorneys may agree to handle business
matters for you; however, the plan rates do not apply to these problems.
You should call the member service department for details on legal Club
of America's Small Business legal Plan if a business related legal need
arises.
You have been assigned an attorney based upon your geographic location,
language preference and specific legal need (if given). If you wish to
use another attorney in the Club, simply contact member services so they
can refer you to another participating attorney. Attorneys are listed by
location, area of practice and foreign language capability.
There are participating attorneys available to handle your problems in
almost every area of the law. These include: Family Law, Personal Injury,
Criminal Law, Real Estate, Wills and Trusts, Estate Planning, Traffic and
D.U.I. related offenses, Incorporation, Worker's Compensation, Employment
Law, Social Security Law, Landlord/Tenant, immigration, and other legal
matters. Many of the participating attorneys either speak foreign languages
or have translators available for clients requesting this service.
CALLING FOR LEGAL ADVICE
Please be sure to have your membership number handy when calling your plan
attorney for the first time. You must notify the attorney's receptionist
that you are a Legal Club member and give your membership number.
You will then be required to give a brief description of your legal situation
so the attorney with the expertise required for your problem can receive
your call. You and your attorney will then decide whether a face to face
consultation is necessary. This is a personal decision between you and
your attorney and may differ with each new legal situation.
Your initial in-person consultation for new legal matters is a members
privilege with no additional charge. After the initial visit, your cost
will never exceed the published rates outlined in this plan member guidebook.
You and your attorney will discuss which payment option is applicable to
your personal legal situation. Please refer to section
3.
You
will pay your attorney directly for any legal expenses
you may incur in accordance with this guidebook. Each attorney
may have different payment plans, available, please ask
your attorney for their specific requirements. Attorneys
may charge a retainer fee at the discounted rates.
CONFIDENTIALITY
You can rest assured that all of your legal matters are guaranteed to be
exclusively between you and your plan attorney. Legal Club of America® will
not interfere in any way with the attorney/client relationship that you
have established. Participating attorneys are obligated by their State's
Professional Code of Ethics to maintain the utmost confidentiality in their
attorney/client relations.
For quality control purposes, you, as a plan member, agree that participating
attorneys may furnish the Legal Club of America® with anonymous information
concerning billing, area of practice, and time spent. This statistical
information allows us to make sure that you are receiving the full benefit
from your membership.
FAMILY ELIGIBILITY
Upon enrollment, you become a plan member of the Legal Club of America.
Your immediate family may utilize your plan privileges in the same capacity.
Immediate family members include your significant other and any unmarried
dependent children who are under 23 years of age. Individuals that are
living with you and are categorically dependent upon you for support are
also included. These individuals include mentally challenged individuals
or court appointed dependents. Unless requested by you as administrator
or executor, services to an individual's estate are not available at plan
rates.

FREE ATTORNEY PROVIDED SERVICES
The following services are available
at no charge from your plan attorney. Please read this
section carefully.
A. During normal business hours, you may
call your plan attorney with any new legal matter. A new
legal matter is one about which you have never spoken to
the attorney before. Your attorney will listen to your
problem, advise you about your legal rights, and help you
find a solution to your problem. In some cases further
legal advice may be unnecessary. If not, you may be advised
on how to solve your problem by yourself. If further representation
is necessary, then you and your attorney will decide the
next step. You may be responsible for long distance toll
charges for telephone calls made by your attorney to you
or on your behalf.
B. Before you sign on the dotted line,
bring your legal documents to your plan attorney for review.
Legal documents include: loan agreements, leases, residential
real estate contracts, employment agreements, etc. Your
plan attorney will review up to 6 pages at no charge. Additional
pages may be reviewed or the entire document may be rewritten
at the guaranteed low hourly rate of $75. plus costs. Documents
that are part of an on-going case, require extended legal
service or require third party negotiation are not included
in this section.
C. Phone Calls and Legal Letters often
help resolve legal matters. When necessary, your plan member
attorney will write an initial letter or make an initial
phone call on your behalf. letters, or legal situations
which need further negotiation with third parties, are
part of an ongoing case, and letters related to collection
matters will accrue additional charges. These charges are
not to exceed the guaranteed low hourly rate.
D. In-Person Consultations - for any
new legal problem are available at no fee. When further
meetings concerning the same matter are necessary, your
attorney will inform you that additional charges will accrue
at a rate of no more than the guaranteed low hourly rate.
E. If a simple will fits your needs -
your plan attorney will prepare a simple will or update
an existing simple will for you and eligible family members
at no charge. You are also entitled to have the simple
will updated at no additional cost. A simple will covers
the distribution of personal property and homestead generally
and does not include specific bequests, guardianships,
complex tax matters, trusts, or estate planning. If your
financial or legal situation requires you to have a more
complex will, your attorney can draft the will at a fee
no more than the guaranteed low hourly rate or guaranteed
fixed fee if your will qualifies. Fixed fees are reviewed
later in this guidebook.
F. Before you represent yourself - in
small claims court, contact your plan attorney for advice
and strategy on how to plan and prepare your case at no
cost. Your attorney will counsel you over the phone during
this initial consultation about how to prepare the necessary
paperwork to represent yourself. If necessary, your plan
attorney may prepare the paperwork for you at an additional
fee in accordance with the discounted rates.
G. When government red tape - has you
going around in circles, your plan attorney will assist
you during an initial consultation in locating the proper
agency and counsel you in how to cut through the red tape
of government programs like Social Security and Medicare
at no cost. Your plan attorney can formally file for services
and represent you at no more than the guaranteed low hourly
rate.

GUARANTEED DISCOUNTS AND HOURLY RATES
The following are special privileges
that your plan member attorney can provide in addition
to those discussed in Section 2.
You and your attorney will discuss which of these options is applicable
to your situation. The plan member is responsible to the attorney for all
expenses incurred, including legal fees, filing fees, costs, and legal
bills of family members using the plan.
These special privileges are:
GUARANTEED LOW HOURLY RATES
GUARANTEED DISCOUNTED CONTINGENCY FEE PERCENTAGES
GUARANTEED LOW FIXED FEES
You may be required to pay a retainer in advance of any filing fees, attorney
fees, or other costs. Also, you may be billed for costs such as photocopying,
postage, long distance toll charges, travel expenses, etc. Once retained,
your plan attorney can provide you with full legal representation including
office work, in-court appearances, investigation, research, drafting, negotiation,
appeals, etc.
Please read this section carefully.
GUARANTEED LOW HOURLY RATE
After the initial no cost consultation, you and your
attorney may decide that further legal work or representation
may be required. If this is the case, you and your attorney
may agree to handle the matter at an hourly rate. Family
Plan members are privileged to have a guaranteed low hourly
fee not to exceed $75 per hour. The attorney can often
estimate how many hours any given case may take. All costs
and filing fees are additional.
If services have already been incurred on the guaranteed
low fixed fee schedule outlined in this section, a matter
may thereafter become too complex than originally anticipated.
If a matter is no longer qualified under the guaranteed
low fixed fee schedule, you may switch to the hourly rate
of no more than $75 for attorney services.
For example, a simple uncontested divorce may suddenly become a contested
nightmare. If the attorney had already worked for many hours on the divorce,
you would pay the guaranteed low fixed rate in the schedule of $210 as
agreed and then no more than $75 per hour for the remainder of the work
needed to resolve the matter. Of course, you and your attorney must be
in full agreement of these fees.
GUARANTEED DISCOUNTED CONTINGENCY FEE
PERCENTAGES
Attorneys often work on a contingency fee basis in matters dealing with
personal injury damages and collections. In these cases as well as others,
the attorney agrees to be paid a percentage of the recovery or award rather
than a set rate or hourly fee.
In some cases, contingency fee maximums are set by state law. In these
matters, your fee will be the lower of either 10% less than the maximum
fee set by state law or the attorney's usual fee. For example, if your
state has a fee maximum of 30%, you would pay the lower of either 27% or
the attorney's usual fee. If the attorney would normally charge 25%, you
would pay this percentage because it is the lower rate.
Where state law does not set maximum levels for contingency fees, your
guaranteed discounted rate will not exceed 29% if settled before trial,
36% if awarded during or after trial, or 40% if settled or awarded after
an appellate brief is prepared.
Guaranteed Low Fixed Fees - On the
following selected services, the Legal Club Plan member

low fixed fees. Please read the definitions section of
this guidebook carefully to see whether your legal problem
will qualify for the guaranteed low fixed fees. These prices
are for the legal fees only and do not include any applicable
filing fees or costs.

If your problem does not qualify for the fixed fee rate,
you and your attorney can agree to the guaranteed low hourly
fee. Your attorney can even estimate the amount of hours
it will take to resolve your problem.
Criminal law matters are usually handled by attorneys at fixed rates. The
rates quoted to members by participating attorneys vary due to the differing
facts and circumstances of each case. The attorneys base these fixed fees
upon an estimate of how many hours of work and expenses that any given
case may consume in conjunction with the low hourly plan rate. Retainer
fees may also vary depending upon the severity of the case at hand and
the jurisdiction where the case is located.

DEFINITIONS & MISCELLANEOUS INFORMATION
DEFINITIONS
Please read this section carefully to be sure of
all of your benefits.
BANKRUPTCY CHAPTER 7 includes preparation
of the petition, documents, and schedules. Filing fees,
court appearances and costs are additional. Attorneys may
charge one fixed rate which would include the basic fixed
fee plus costs, court time and expenses.
CORPORATION (REGULAR) includes preparation
of the incorporation documents, articles of incorporation,
by laws, and the minutes. Does not include filing fees,
costs or the corporate kit. Any extended work for the new
corporation is not eligible for family plan discounts.
DIVORCE (SIMPLE) is when the plan member's
spouse is not represented by separate counsel, there are
no minor children under age 18, marital assets are less
than $70,000 and all issues are agreed to without aid of
counsel. This does not include court appearances, the filing
or preparation of documents affecting property or costs,
or separation agreements.
ELIGIBLE FAMILY MEMBERS are the plan
member's significant other and unmarried dependent children
household who are under the age of 23 and any categorically
dependent individuals living in the plan member's home.
NAME CHANGE obtaining name change for
plan members and eligible family members where it is not
contested or challenged. This includes preparation and
filing of documents only. Court representation,
filing fees and costs additional.
NON-COMMERCIAL REAL ESTATE CLOSING includes
reviewing purchase or sale agreements, one hour of telephone
counseling, and a one hour appearance at closing for the
purchase or sale of residential real estate not used for
business or investment purposes. Title policy, other documentation
and extended counseling (if any) is an additional charge.
NON-SUPPORT (SPOUSE/CHILD) includes the
preparation for and the attendance at a single hearing
for judgments or contempt citations for non-payment of
alimony, child support or maintenance and one hour of collection
efforts after the hearing. Preparation does not include
obtaining or discovering evidence, extended court time,
and extended collections work.
PLAN/PARTICIPATING ATTORNEYS are licensed
and qualified to practice law in your state, are required
by the company to maintain professional liability insurance,
and have contracted with Legal Club of America to provide
legal services to plan members as outlined in this Plan
member Guidebook for the law areas that they handle and
the cases that they accept.
PLAN MEMBER is any person who has contracted
with the Legal Club of America for a membership in the
plan and has a current paid membership.
SIMPLE WILL (NO CHARGE WILL) a will
distributing personal property and homestead generally
and not involving trusts, specific bequests, real estate,
tax matters, guardianships, living wills, health care proxy,
or partitions.
TRAFFIC DEFENSE generally includes the
preparation of court documents and attendance at a one-time
only court hearing to settle the issue. Does not include
court costs, fines, additional hearings or other related
expenses. Does not apply in jurisdictions that consider
traffic offenses to be criminal matters.
WILL WITH SIMPLE MINOR'S TRUST a simple
will with a minor's trust for the surviving minor children
of the planmember. This will is eligible for the guaranteed
low fixed fee. It does not cover other kinds of trusts,
complex tax matters, administration or estate planning.
These additional estate planning matters can be handled
under the guaranteed low hourly rate.
MISCELLANEOUS INFORMATION
Participating attorneys are bound by their state's professional code of
ethics. They will advise you if they have a conflict of interest in taking
your case. Attorneys have the inherent right to decline a case for any
reason. If this occurs, you may be referred to another attorney on the
plan by calling the customer service center of Legal Club of America® for
assistance.
The Legal Club of America® reserves the right
to terminate your membership if you are delinquent in paying
your membership fee. Also, Legal Club of America® may cancel
your membership only after ten days notice to you. Further,
you may cancel your membership at any time by written notification
only. Membership dues are fully refundable for thirty (30)
days following the initial enrollment unless otherwise
contracted.
Membership fees, terms, and fees charged
by plan attorneys are subject to change only after prior
notice to the plan member.
Attorney fees are paid directly to your plan
attorney. Participating attorneys are not employees of
Legal Club of America and have no financial obligation
to the company.
Legal Club of America® is not liable to indemnify
or reimburse any plan member or participating attorney
for any attorney fees or costs generated by the plan member.
The plan member is to use the plan for personal
and family legal services only and not for businesses owned
by the plan member or for investment matters. Legal work
for any corporation or business entity is available at
discounted rates through the legal Club of America Small
Business Legal Plan.
The penalty for deceiving the Legal Club
or any plan attorney for the purpose of utilizing the plan
for a non-eligible family member is automatic termination
of plan privileges and immediate notification to the plan
attorney about the termination.
Legal Club of America® Corporation, its
subsidiaries, State Bar Associations and other regulatory
agencies do not guarantee the quality or quantity of legal
services that are provided by plan attorneys. However,
all participating attorneys are checked by the company
with the attorney's respective Bar Associations periodically,
not less than annually, to verify that they are in good
standing.
The plan attorneys in their attorney/client
relationship have the sole responsibility for providing
legal services to the plan member.
Legal Club of America® Corporation, is not
a law firm, insurance carrier or a provider of legal services.
Plan members have the right to file a grievance concerning any attorney's
professional conduct with the proper regulatory authority in their state.
All memberships shall automatically renew
at the end of each membership term, unless Legal Club is
notified in writing at least thirty (30) days prior to
the end of the term.
The term "guarantee" as used in this guidebook
refers to the guarantee that Legal Club of America® will
find and refer its members to an attorney that will abide
by the fee schedule outlined herein. If Legal Club of America® cannot
find such an attorney for a member, the member's only recourse
is a refund of their membership fee.
Legal matters in states where Legal Club
is not available. Plan privileges and pricing schedules
may vary by State in accordance with regulatory authority
mandates.
Court filing fees, expert witness fees, court
costs, court reporter fees, transcript expenses, photocopying
costs, postage, telephone toll charges and any other incidental
expenses incurred by the plan member are excluded from
discounted rates under any of the three pricing formulas
described in this guidebook. Travel expenses are not eligible
at discounted rates when the attorney must travel to represent
a plan member's interests.

EXCLUSIONS
The following matters are excluded
from your plan privileges:
Legal matters involving the laws of jurisdictions outside the United States
or its subdivisions.
Legal matters where the plan member has already retained participating
counsel at their usual rates prior to enrollment in the Legal Club.
Frivolous legal matters as determined by the plan attorney in accordance
with the professional code of ethics in their state.
Any action involving Legal Club of America®, plan attorneys, affiliated
companies or any of their company's or affiliated company's directors,
officers, employees or agents in any matter in which they have interests
adverse to the plan member's.
Legal matters against the plan sponsor, employer, directors, officers,
agents or employees, where Legal Club membership was gained through the
sponsor or employer's efforts.
Legal matters related to personal or family matters.

This
section is provided in order to introduce our members to
some of the commonalties that exist with the legal system.
Many of our members have never used the legal system and
as such occurrences that are common to some are foreign
to most. Below is a list of common occurrences and resolutions.
Remember that many problems experienced by our members are simply miscommunications
between them and the attorney's office. If you are unable to work a problem
out on your own, we invite you to call Legal Club of America® for an immediate
solution. You can reach Customer Services at (800) 305-6816, Monday through
Friday, 9:00am-8:00pm EST.
Why has my attorney not called me back?
This is a very common occurrence. Attorneys
are often in deposition or court and as such they may not
call back for two or three days. In order to remedy this,
the HR Director should ask the employee when they called
the attorney and whether or not they have a court date
in the next 2-3 days. If so, the employee should call LCOA
for immediate intervention and/or reassignment.
Why
does the attorney or individual answering the phone
say they have never heard of Legal Club of America® or
that they do not know if they are a part of the program?
This problem is generally a misunderstanding as a result of a new secretary
answering the phone. In addition, many attorneys, like doctors, are part
of several plans and as such they sometimes forget the names of all of
them, or the fee structure involved. Simply call Legal Club of America® for
immediate intervention and/or reassignment.
I believe the attorney assigned to
me is too far away.
This is generally not a problem at all.
Members need to understand that most legal care can be
handled over the phone, by fax or email. If you need to
visit the attorney, a one-hour drive to save thousands
of dollars, once or twice a year is well worth it. This
is truly a case of not being familiar with the legal system.
Unlike doctors, who must look at you for treatment, the
same is not the case with attorneys. If a local attorney
is not available and is needed for a court matter, the
Legal Club of America legal staff will scout the area to
recruit one. In the mean time, the member can call the
attorney assigned for counseling and services.
The attorney is not following the fee
schedule.
Attorneys are often part of several plans
and as such they confuse fee schedules. First, make sure
that you identify yourself as a Legal Club member. If the
fee schedule is still not followed, call us immediately
so that we can intervene, reeducate the attorney and get
things moving in the right direction. If the attorney is
not cooperative we will immediately reassign you to another
attorney. Our Legal Department professionally manages our
nationwide network of attorneys and every quarter "Professionalism
Checks" are performed on all of our attorneys. This communication
helps to maintain the attorney's awareness of your exclusive
fee schedule as a Legal Club member.
The attorney was not nice.
Unfortunately, attorneys can be abrasive.
This is not always true but it does happen. Realize that
this aggressive and perhaps intimidating attitude may also
play to your advantage if it comes time for that attorney
to represent you in whatever capacity you require.
My Plan attorney won't take my case.
Accepting a case is left to the personal
judgement of the attorney. The attorney may determine that
the case lacks merit in which case, they are truly trying
to save the member the time and money of pursuing a potential
losing case. In addition, the attorney may be too busy
to give the member the time and care they require. In either
situation, members should call Legal Club of America® and
request another attorney. They may use as many plan member
attorneys as they like. Results with the next attorney
could differ.
Communications
Remember that the majority of perceived
problems are usually miscommunications between the member
and the attorney's office. A call to Legal Club of America® can
always solve this. Follow the guidelines above and first
try to solve any problems on your own. However, when in
doubt or if your problem persists, call Customer Services
at (800) 305-6816, Monday through Friday 9:00am-8:00pm
EST.

Q. - Can I change my attorney and how often?
A. - Yes. As often as you like, without limit.
Simply contact Legal Club's customer service department
at (800) 305 - 6818.
Q. - Must I always use the attorney referred to
me in my welcome letter?
A. - No. This attorney either matches the criteria
you asked for in your initial application and/or is a general
practitioner that can usually assist you in completing
your Free simple will, as well as consult with you on many
different legal issues. You may change your initially referred
attorney for any reason, at any time, and make use of as
many attorneys in the network as you like.
Q. - How close will my attorney be to my home
or office?
A. - In all cases we try to refer an attorney
to a member that is as close as possible. We guarantee
a 30 mile limit. It is well worth the trip if a member
can save thousands of dollars. Additionally, a great deal
of the legal care required by members can be handled over
the phone, fax and/or by email. Items such as document
review, consultations and Free simple wills are perfect
examples.
Q. - Do plan attorneys have to take my case?
A. - No. A plan attorney, after providing you with a free consultation
may determine that your particular legal issue does not warrant any further
action. If you disagree with the plan attorney, feel free to call Legal
Club for another attorney referral for another legal opinion. You may
do this as often as you like. In other cases, the law firm or attorney
you were referred to may simply be too busy to give your situation the
proper attention it deserves. If this happens, they have been instructed
to have you call Legal Club for a new referral.
Q. - Can plan attorney ask me for a retainer?
A. Yes they can. Upon completion of a free consultation,
you and your attorney may decide to proceed with further
legal action. This could require a substantial amount of
hours being spent on your case by your plan attorney. If
such a situation arises, your plan attorney may ask for
a retainer based on the estimated number of hours he or
she thinks the case will take. As a plan member the retainer
will be calculated using the rate of $75.00 per hour.
Q. - What can't I use the plan for?
A. - The services of your attorney can not be
used to adversely affect your employer, the agent who sold
you this program or Legal Club of America.
Q. - Is this plan insurance?
A. - No. Our plan is a Group Legal Service program
that is not insurance in any of the 50 states.

Our Identity Theft Restoration Administrator’s core

| If an LCA member becomes a victim of identity theft, the plan administrator assigns a Recovery Advocate to manage the case and activate the members of our Recovery Team (fraud investigators, legal counsel, and other experts) needed to help with the recovery process from beginning to end. This includes gathering the facts for a Damage Assessment and creating a customized Recovery Plan. The Recovery Plan outlines the actions the member and Recovery Team will need to take to complete the recovery process. |
Examples of recovery services
- Provide Customer with personalized damage assessment
and recovery plan. Set goals for recovery process
- Notify the three major credit bureaus of fraud, place
fraud alerts, and request complimentary credit reports
- Review the Customer’s credit files with the Customer
to determine the accuracy of the file and potential areas
of fraud
- Notify the Customer's affected creditors, financial institutions,
and utility providers of the identity theft (available
on an hourly rate basis)
- Provide assistance with filing a Police Report/locating
office
- Collect information regarding misuse of the Customer’s
accounts - to be shared with law enforcement
- Create and maintain a case file to document the identity
fraud
- When appropriate, provide assistance with obtaining and
reviewing the Customer's Social Security Personal Earnings
and Benefits Statement

How often can I use my Dental/Vision?
You can use your plan as often as you like.
How long will it take to receive my card?
Online Orders are usually entered into our system
within two business days. You will be able to use your Dental
benefits as soon as you are entered into the network database
(usually within 3 business days). Expect to receive your MembersNetwork Dental/Vision card within 7-10 days.
What
do you do with the information I supply with an online order?
The information you supply is used strictly for access to
our networks. Any information that you supply is kept confidential
and will not be sold or used for any other reason. If you
don't want to provide your social security number, simply

not mean you are getting lesser service or quality. Everything

Is your plan a health insurance plan?
No. These plans provide discounts and savings for health care
expenses such as dental, vision, hearing, chiropractic, and
prescription drugs charges through participating providers.
What else will I receive with my membership
card?
You will receive a complete package in the mail detailing

What if I'm not satisfied with the
plan coverage?
If you are not satisfied with your plan, you may cancel your
MembersNetwork plan within the fist 30 days and receive your

Can I use your discount plans to supplement
my insurance benefits?

yes, you may use it to provide discounts on services not covered.
However, you may not use this plan with any other discounts
you might receive. (For instance, in the event you have a
set co-pay, you cannot use this plan to provide a discount
in addition to the co-pay.) It is still important to note
that our dental/vision plans are NOT AN INSURANCE PLAN, nor

In most cases, you will pay a pre-negotiated fee for services
rendered. In other cases, you will receive a discount on items,

How can I check to see if there are

How will a provider know if I am enrolled
